Zadani Kifoza

Kifoza predstavlja deformitet kičmenog stuba sa posteriornim izbočenjem, najčešće u torakalnom delu kičme.



Uzrok nastanka

Najčešće je uzrok kifoze nepoznat (idiopatska kifoza). Postoje razne teorije, kao konstitucionalna slabost skeleta, traumatsko oštećenje zona rasta pršljenova, delovanje zapaljenskih, endokrinih, metaboličkih i drugih faktora.
Ovaj deformitet stiče se najčešće u periodu razvoja, tako da možemo razlikovati tri kritična perioda kada je osoba podložna ovim promenama.
  • Period uspravljanja deteta – uspravljanje deteta treba da bude pirodno, a ne potpomognuto od strane nestrpljivih roditelja. Svako prerano uspravljanje deteta može dovesti do jednog od ovakvih telesnih deformiteta.
  • Period polaska deteta u školu – Dete veliki deo vremana provodi u specifičnim položajima kao što je sedeći položaj u klupi koji često za njega može biti naporan što dovodi do zauzimanja nepravilnog položaja i naravno telesnog deformiteta kao što je kifoza.
  • Period puberteta – period u kome dolazi do burnih promena, između ostalog do bržeg rasta kostiju i nemogućnosti snage mišića da isprati njihovu dužinu, što vrlo lako može dovesti do deformiteta kao što je ovaj.

Klinička slika

To je povijeno (pogrbljeno) držanje, koje treba razlikovato od prave grbe, nastale usled tuberkuloznog oboljenja pršljenova. Povijenost kičmenog stuba, kifoza, koja nastaje kao posledica rahitisa, ili drugih oboljenja u toku detinjstva, lučnog je oblika, dok su tuberkulozne grbe više šiljate. Ovakvo, povijeno držanje, zove se kifotično držanje. Ova povijenost nastaje zbog nedovoljne sposobnosti ligamentarnog i mišićnog aparata da pod određenim uslovima zadrži kičmeni stub u pravilnom položaju.

Dijagnoza

Dijagnoza se postavlja kliničkim pregledom i rendgenskim snimkom. Pregled se vrši u stojećem i ležećem položaju. Funkcionalna kifoza se gubi u ležećem položaju, strukturalna perzistira. Rendgenski snimak se pravi u profilu, uglavnom u stojećem položaju.

Lečenje

Pravilno držanje zahteva dugotrajne, kontrolisane vežbe jačanja leđne muskulature, koja na sebe treba da preuzme ulogu stabilizatora držanja, u težim slučajevima dodaje se i mider ili gips, a u najtežim i operativno.

Prevencija

Treba pre svega, obezbediti potpuno slobodan razvoj deteta, ukloniti sve što mu sprečava slobodne pokrete i ne prisiljavati dete na pokrete koji još nisu za njega. U tom smislu je rano uspravljanje deteta štetno. Dete treba da se uspravlja kada ono samo to odluči. Vrlo je korisno da se dete radi igranja postavlja potrbuške. I puzanje je vrlo korisno, jer jača mišiće koji stabilizuju kičmeni stub. Školsko dete treba redovno da vežba. Izbegavanje časova fiskulture i izvlačenje od ove obaveze pod raznim izgovorima veoma je štetno po razvoj deteta. Vežbanje disanja i igre u slobodnom prostoru od velike su koristi. Za obezbeđenje kičmenog stuba potrebna je jaka leđna muskulatura. U krajevima gde je običaj da se teret nosi na glavi, žene su veoma prave čak i u dubokoj starosti, jer one na taj način ojačaju leđne mišiće.

Zadani Kifoza kicme

Kao sto i sam naziv kaze ima kifozu kicme koja je vidljiva dok sedim i kad pruzim ruke na dole naprimer da nesto podignem.Ali ono sto me interesuje imam 32 god.to mi se pojavilo sa nekih 20-tak godina.Nazalost nisam davao velikog znacaja tome.Do sad kad je pocelo da mi se savija jos vise i poceli bolovi.Naravo icu kod lekara,ali me jedo interesuj dali kifoza moze da naglo predje u veliku pogrbljenost ili tako sa se savije da lici na grbu,i dali moze da se zaustavi ne da se ispravi.Jos nesto kad stojim i probam prstom da vucem preko prsljenova nemogu da napipam jedan prsljen od kojeg pocinje savijanje.

Zadani

Poremećaj zakrivljenosti kralježnice u sagitalnoj ravnini, tj. kifoza, je stanje kod kojega konveksnost torakalnog dijela kralježnice prema nazad prelazi fiziološke granice (>20° do 45°).

Dakle, kifoza se ne odnosi na vratnu kralježnicu nego na torakalni dio.

Kralježnica odraslog čovjeka promatrana sa strane, odnosno u sagitalnoj ravnini ima oblik dvostrukog slova S - lumbalna lordoza i torakalna kifoza.
