Vitamin D je vjerojatno jedini vitamin o kojem se ne govori puno u svijetu nutricionizma.
Možda zbog toga što je vitamin D načelno besplatan: tijelo ga stvara samo u dodiru sa sunčevom svjetlošću.

Upravo zbog toga mnogi ljudi ne znaju koliko je vitamin D važan za naše zdravlje.
Prema najnovijim istraživanjima, vitamin D sprečava osteoporozu, depresiju, rak prostate, rak dojke, utječe na dijabetes i pretilost, a usko je povezan s bolestima štitnjače i nadbubrežne žlijezde.

15 činjenica o vitaminu D

1. Naša koža proizvodi vitamin D kada je izložena sunčevoj svjetlosti - UV zračenju iz prirodnog izvora.
2. Iscjeljujuće zrake sunca, iz kojih koža stvara vitamin D, ne mogu proći kroz staklo. Zbog toga kada se nalazite u kući, uredu ili autu vaša koža ne stvara vitamin D.
3. Skoro je nemoguće dobiti adekvatne količine vitamina D samo iz prehrane. Izloženost sunčevoj svjetlosti je jedini siguran način da dobijete dovoljne količine ovog vitamina, ali to nije tako jednostavno a često i nije moguće toliko opsežno, redovno i dugotrajno izlaganje suncu.
4. Što dalje živite od ekvatora, tim duže trebate biti izloženi suncu da bi dobili dovoljno vitamina D.
5. Bilo bi potrebno dnevno popiti 10 čaša mlijeka, obogaćenog vitaminom D, da bi unijeli minimalne količine ovog vitamina u organizam.
6. Ljudi sa tamnom kožom trebaju 20-30 puta više sunčanja, da bi dobili istu količinu vitamina D kao ljudi bijele kože.
7. Dovoljne količine vitamina D su presudne za apsorpciju kalcija u organizmu. Bez vitamina D naše tijelo ne može apsorbirati kalcij, što čini dodatke prehrani s kalcijem beskorisnima ako imate manjak vitamina D.
8. Kroničan nedostatak vitamina D ne možete riješiti preko noći. Potrebni su mjeseci i mjeseci izlaganja suncu ili uzimanja vitamina D u obliku suplementa, da bi povratili ovaj vitamin u kosti i živčanom sustavu.

Novija istraživanja o povezanosti vitamina D i Hashimota će vam dati i dodatan razlog za sunčanje.
Vitamin D ima važnu ulogu u regulaciji imunološkog sustava. Istraživanja su otkrila izravnu vezu između nedostatka vitamina D i bolesti imunološkog sustava (multipla skleroza,dijabetes tipa 1) a znanstvenici iz Turske su predočili dokaz da nedostatak vitamina D vodi autoimunom oboljenju štitinjače (Hashimoto).

Nizak vitamin D „aktivira“ Hashimoto?

Istraživanjem na 540 ljudi,znanstvenici su uspoređivali nalaze vitamina D 25-hydroxy sa antitijelima štitnjače (anti-TPO i anti-TG). Otkrili su da pacijenti koji boluju od Hashimota imaju niže razine vitamina D u usporedbi s oboljelima od samo hipotireoze i zdravim osobama u trećoj kontrolnoj skupini.
Također su došli do otkrića da su razine vitamina D izravno povezane sa veličinom štitinjače (oboljeli od Hashimota često imaju problem sa povećanom štitnjačom). Ovo istraživanje je pokazalo da je razina vitamina D povezana sa razinom antitijela, što su antitijela viša, razina vitamin D je niža. Rezultat ovog istraživanja ukazuje na moguću poveznicu između nedostataka vitamina D i obolijevanja od Hashimota. Također je moguća poveznica između razine nedostatka vitamina D i napredovanja daljnjeg oštećenja štitinjače.

Dobivam li dovoljno vitamina D?

Mnogo je razloga zašto je dogovor ne. Kronični stres, pestidicidi pa čak i plastika mogu sniziti razinu vitamina D u tijelu. I dok nam se govori da možemo dobiti dovoljno vitamina D iz Sunčeve svjetlosti, to baš i nije potpuno istina.
Sunčeva svjetlost proizvodi UVA i UVB zrake i dok UVB pomažu proizvodnji vitamina D, UVA razbija taj isti vitamin D. Kreme za sunčanje također blokiraju vitamin D.

Jedini način da saznate imate li optimalan nivo vitamina D je test iz krvi. Većina ljudi uopće nemaju simptome nedostatka, dok su kod drugih simptomi nejasni, uključuju umor, osip i bolove te debljanje u struku i nemogućnost skidanja kila.

Kako dobiti više vitamina D?

Naravno da je odgovor Sunce ali i kroz hranu: ulje jetre bakalara, masna riba, goveđa jetra, kamenice, jaja, gljive, žitarice i mliječni proizvodi.
Ali najbolji način za dostići optimalnu razinu vitamina D su suplementi. Dnevno uzimanje od 5000-10 000 IU može napraviti veliku korak prema dostizanju optimalne razine 40-60 ng/mL,a najviša gornja granica je 80 ng/mL.

Napomena: u hrvatskih labaratorijim se koristi jedinica mjere : nmol/L.
- 80 ng/mL - 200 nmol/L
- 40 ng/mL – 100 nmol/L
-60 ng/ml – 150 nmol/L


Vitamin D i rak

Vitamin D je pokazao impresivne rezultate kod ishoda ozbiljnih bolesti jer djeluje protuupalno .
Prije 30ak godina,postojala je hipoteza da nedostatak vitamina D povećava rizik od raka debelog crijeva. Od tada su gomile istraživanja išla u koristo ove hipoteze a istraživanje iz 2006-te godine otkriva rezultate 25-godišnjeg istraživanja da povećanje razine vitamina D u krvi do razine 35 ng/ml,smanjuje rizik od raka debelog crijeva za čak 50 posto!

Još jedno vrlo impresivno istraživanje iz 2008 godine je uključivalo 1178 žena poslije menopauze koje su bile podijeljene u tri skupine. Prva skupina žena je uzimala suplement kalcija,druga skupina kalcij i 1100 IU vitamina D i treća skupina je bila placebo skupina.
Na kraju četvrte godine ,druga grupa koja je uzimala kalcij i vitamin D,imala je 77 % smanjenu učestalost svih vrsta raka.
Istraživanje dr Cedrica Garlanda je pokazalo da povećanje razine vitamina D do 40-50 ng/ml može smanjiti rizik od raka dojke za 50 %.
Muškarci sa rakom prostate i visokim vitaminom D ,u usporedbi sa muškarcima sa istom bolesti ali niskim vitaminom D,su imali 60 - 85 % smanjen rizik od smrtnog ishoda zbog bolesti prostate. To je skoro 7 x veći rizik smrtnog ishoda od raka prostate za one sa niskom razinom vitamina D.

U istraživanjima raka,ovo su impresivni podaci i nijedna druga supstanca nije pokazala ovakve značajne rezultate kao vitamin D.
Norveško istraživanje iz 2012-te godine je pokazalo da nakon 9 godina praćenja oboljelih od raka pluća,debelog crijeva,limfoma i raka dojke,sa visokom razinom vitamina D,imaju 66 % manju šansu od smrtnog ishoda naspram onima sa niskim vitaminom D.


Vitamin D i bolesti srca

Vitamin D je povezan i sa kardiovaskularnim bolestima. Njegova uloga u tijelu je utjecaj na krvni tlak i prevenciju oštećenja arterija. Jedno veliko istraživanje na 50 000 zdravih muškaraca u razdoblju od 10 godina,je otkrilo da muškarci sa niskim vitaminom D imaju dva puta veći rizik od srčanog udara.

Iznimno je važno znati ako suplementirate vitamin D - da on uvijek ide u paru s vitaminom F. Suplementacija visokim količinama vitamina D a bez dovoljno vitamina F i ako imamo premalo žući (ako nam jetra ili žuć ne rade pravilno ili nam je uklonjen žući mjehur) dovodi do vrlo opasne hipervitaminoze. Vitamin D is an important vitamin that not only regulates calcium, but also has many other beneficial actions. Not many endocrinologists realize this, but several articles published over 20 years ago showed that patients with hypothyroidism have low levels of vitamin D.
This may lead to some of the bone problems related to hypothyroidism. It was thought that one of two mechanisms may explain the low levels of vitamin D in patients with hypothyroidism, 1) the low levels of vitamin D may be due to poor absorption of vitamin D from the intestine or 2) the body may not activate vitamin D properly. Other articles have demonstrated that patients with Graves disease also have low levels of Vitamin D.

Importantly, both vitamin D and thyroid hormone bind to similar receptors called steroid hormone receptors. A different gene in the Vitamin D receptor was shown to predispose people to autoimmune thyroid disease including Graves’ disease and Hashimoto’s thyroiditis. For these reasons, it is important for patients with thyroid problems to understand how the vitamin D system works.

Sources of Vitamin D
Vitamin D is really two different compounds, ergocalciferol (vitamin D2), found mainly in plants and cholecalciferol (vitamin D3), found mainly in animals. Both of these hormones are collectively referred to as vitamin D, and they can either be obtained in two ways. One is by exposure of the skin to the ultraviolet (UV) rays of sunlight or also from dietary intake.

Vitamin D is found naturally in fish (such as salmon and sardines) and fish oils, eggs and cod liver oil. However most Vitamin D is obtained from foods fortified with Vitamin D, especially milk and orange juice. Interestingly, as breast feeding has become more popular, the incidence of Vitamin D deficiency has increased as less fortified milk is consumed.

Vitamin D deficiency may also occur in patients with malabsorption from their intestine, such as in the autoimmune disease called Celiac Disease, which occurs frequently in patients with thyroid problems.

Different Forms of Vitamin D and How To Diagnose Vitamin D
Vitamin D itself is inactive and needs to get converted to the liver to 25-hydroxy vitamin D (25-OH vitamin D) and then in the kidney to 1, 25-hydroxy vitamin D. It is only the 1, 25- OH vitamin D which is biologically active. This form of vitamin D acts to allow for absorption of calcium from the intestinal tract. Therefore, patients with low vitamin D levels will have low calcium and in severe cases get rickets (in children) or osteomalacia (in adults) which is when the bone bows out and is poorly formed. In mild cases of vitamin D deficiency, osteoporosis occurs, but low calcium is rare.

The conversion from the 25-OH vitamin D to the 1, 25-OH vitamin D that occurs in the kidney is catalyzed by parathyroid hormone, also called PTH. Therefore, patients with very low vitamin D levels will have relatively high PTH levels often with low calcium levels.

This is similar to patients with primary hypothyroidism having elevated TSH levels while having normal thyroid hormone levels. Additionally, the 25-OH vitamin D form which is the storage form and is much more abundant that the 1, 25-OH vitamin D form which, although is active, is less abundant. Therefore, in states of vitamin D deficiency, low levels of 25-OH vitamin D are found, but the 1, 25-OH vitamin D levels are either normal or actually slightly high. They are slightly high because the excess PTH that is stimulated by the low 25-OH vitamin D levels stimulates the conversion up to 25-OH vitamin D to the 1, 25-OH vitamin D. Thus, patients that are vitamin D deficient usually have a low 25-OH vitamin D level, a high PTH level, a low normal calcium, and a normal or an elevated 1, 25- OH vitamin D level. However some patients may actually have a high normal calcium as the elevated PTH and 1, 25-OH vitamin D may cause increased calcium absorption from the GI track and reabsorption from the kidney. If 25-OH vitamin D levels were not measured, these patients might have been incorrectly diagnosed with mild hyperparathyroidism as they have a high normal calcium and a high PTH.

Dr. Friedman usually recommends measuring a 25-OH vitamin D to determine if a patient does have vitamin D deficiency. Occasionally, PTH and calcium, can also be ordered. The 25-OH vitamin D assay has a normal range of approximately 20-60 ng/dL. However, this range may be too low for many patients. In general, Dr. Friedman would recommend treatment of patients that have a 25-OH vitamin D of less than 30 ng/dL. Optimal levels of 25-OH Vitamin D for patients with thyroid diseases are probably 35-60 ng/dL.

Vitamin D appears to have many effects besides being related to calcium and bone health. Some patients with low vitamin D levels have fatigue and bone pain, which is easily reversible with proper replacement of vitamin D. Vitamin D may protect against heart disease and some types of cancer. Vitamin D may also have some role in regulating the immune system and also reducing blood sugar levels in patients with diabetes. Proper vitamin D levels are needed to prevent osteoporosis. In conclusion, proper vitamin D levels are essential for one’s health, especially if you have thyroid problems. Unless a patient is exposed to sunlight or foods containing vitamin D, screening for Vitamin D deficiency is recommended for all thyroid patients.

There are over 30,000 papers published citing the importance of Vitamin D for our physical and mental health. Such research cites that Vitamin D can improve pregnancy outcomes, reducing risk of Cesarean section and preeclampsia, lower risk of type 1 and 2 diabetes, lower risk of heart disease and stroke, improve Alzheimer’s, autism or other brain dysfunctions, and fight off viral or bacterial infections.

Researchers found calculation error that drastically changes Vitamin D dosage recommendations.

If Vitamin D can do these things, why do we have so many people struggling with these very issues? New research coming from UC San Diego and Creighton University believe that the recommended intake for Vitamin D is not accurate and are requesting that this amount be changed. (2)

A letter that was published two weeks ago by investigators confirmed that there was a calculation error. Dr. Cedric Garland, adjunct professor at UC San Diego reported, “The error has broad implications for public health regarding disease prevention and achieving the stated goal of ensuring that the whole population has enough vitamin D to maintain bone health.” (2)

Daily recommendation for Vitamin D should be 7,000 IU/day.

The recommended Vitamin D intake is 600 IU per day through the age of 70 and 800 IU daily for those who are older. Calculations by us and other researchers have shown that these doses are only about one-tenth those needed to cut incidence of diseases related to vitamin D deficiency.

Robert Heaney, M.D., of Creighton University wrote: “We call for the NAS-IOM and all public health authorities concerned with transmitting accurate nutritional information to the public to designate, as the RDA, a value of approximately 7,000 IU/day from all sources.”

This intake is well below the upper level intake specified by IOM as safe for teens and adults, 10,000 IU/day.

If you are vegan, eating foods that contain Vitamin D3 is not realistic as these foods are beef liver, cheese, egg yolk and fatty fish. It’s important to soak up as much sun as possible and take a high quality supplement.

Baš sam se ovih dana malo bavila ovim vitaminom i doznala da je kod suplementacije važno poštovati sinergiju vitamina D s vitaminom K2 i s magnezijem. D navodno troši puno magnezija.

Može li se pretraga 25-OH vit D napraviti na uputnicu? Ima li tko iskustva s time?

I ima li tko iskustva sa suplementacijom D vitaminom? Počela sam ga uzimati prije tjedan dana, ali još ne osjećam ništa

Ja sam napravila pretragu na D vitamin ali ne znam koji oblik gledaju. U svakom slučaju bio je nizak. Pretraga se radi na uputnicu, znam da rade u Vinogradskoj.

S obzirom da je bolje uzimati D3 - aktivni oblik vitamina (za razliku od D2 oblika), onda mislim da nije toliko bitno ovo s 25-OH, a ionako pijem i magnezij, tako da se ne opterećujem previše. Gledala sam i ovo za K2 i mislim da omjer treba biti 8-10:1 ali još nisam krenula s tim (iako imam u planu, dobro si me sjetila). Ima već gotovo smiksanih proizvoda D3+K2 tako da se ne mora voditi računa o omjeru. To je praktičnije.

Odnedavno sam s 1000 UJ povisila na oko 5000 UJ i iskreno ne primjećujem ništa ... uzimam već 4-5 mjeseci, ali iskustva drugih govore da je kod niske razine D vitamina bilo potrebno i do godinu dana uzimanja i do 10.000 UJ dnevno da se sanira manjak i stabilizira stanje u organizmu.
